Roper St. Francis Healthcare is dedicated to extending high quality, convenient care to every corner of our community, that includes outside of the traditional hospital setting. Our Remote Patient Management program uses technology to connect with patients in their homes to provide support, education and oversight of their condition.

How it works

Upon enrollment in the program, you will be enrolled in a care plan tailored to your healthcare needs. The clinical team oversees your vital signs remotely, escalating to an advanced practice provider (APP) or physician when necessary. Here’s what to expect:

  • Daily Monitoring: Our clinical team conducts daily rounds to keep an eye on your reported vital signs such as activity level, blood pressure, heart rate, oxygen saturation, temperature and weight.
  • Symptom Surveys: Share your wellbeing through symptom surveys, providing the clinical team with essential insights into your health.
  • Personalized Education: Receive education specific to your condition(s) to help guide you in following your care plan.

RPM is used to treat various chronic conditions including, but not limited to:

  • Cardiology and Cardiovascular Services (CHF, Hypertension, Cardiac Surgery, A-Fib)
  • Pulmonology (COPD, Asthma, Pneumonia)
  • Endocrinology (Diabetes)
  • Chronic Care Management (CCM)

Equipment and technology

During the program you can expect to use one or more of the following devices:

  • Tablet or mobile application for your smartphone
  • Blood pressure cuff
  • Pulse oximeter
  • Weight scale
  • Thermometer
  • Glucometer (if required)

Patients will be taught the importance of taking and understanding their vital sign readings, with the support of their provider and the Telehealth Care Center.

    Yes, Remote Patient Monitoring platforms prioritize patient privacy and data security. Data transmission is encrypted to protect patient information, and strict protocols are followed to ensure compliance with healthcare privacy regulations, such as HIPAA (Health Insurance Portability and Accountability Act).

  4. How does enrollment for the RPM program work?
    - Referral: Obtain a referral from a physician or APP.
    - Assessment: Undergo an assessment to determine your suitability for the program.
    - Agreement: Once approved, you will agree to join the program.
    - Setup: You’ll be set up with the necessary equipment and a personalized care plan.

  5. How long will I be enrolled in the RPM program?

    Length of the program will depend on your provider’s recommendation but could be anywhere from 16-30 days, or longer if needed.


  6. Will I be trained on how to use the RPM equipment?

    Yes. Our team will ensure you are comfortable and confident with using the RPM equipment and devices by providing education and support.


  7. Can I connect with my care team using the RPM technology?

    Yes. The technology allows for audio calls, video calls, and text messaging between the patient and care team.


  8. Are there costs associated with being enrolled in RPM?

    Yes. The costs of participation in an RPM program vary depending on insurance company and coverage. You can anticipate a copayment, similar to what you pay at a physician office visit. We recommend contacting your insurance provider for more information prior to enrolling.
